The following requirements must be met by the applicant (the person with a disability) to qualify for the issuance of a City parking permit for people with disabilities.

Must be a resident of New York City or a non-resident employed full-time or attending school full time in New York City.

If you are non-resident, you must ask your employer or school to submit a letter on original company, organization, or school letterhead verifying that you are employed or attend school full time in New York City. The letter should be signed by an official of the company/organization/school and, if a school, should be embossed with the school seal. Mail a copy of the letter to:

    Must require the use of a private automobile for transportation.

You may list up to ten plate numbers on your permit. Vehicle registration must be current and valid for each listed plate.

If a plate is registered outside of the State of New York, you must mail a copy of the vehicle registration to:

You are not required to mail a copy of the vehicle registration for plates registered within the State of New York.

    On the application to follow, you will be required to supply general personal and vehicle information. Once you have filled all necessary fields, you will be allowed to print out the application and deliver to your personal physician.

Remember to include all supporting documents (see the Residency Requirements and Vehicle Requirements sections above).

If your physician submits your medical history information online and you have no supporting documents to submit, you need not mail the printed application.

  1. Once you submit your application, it will be reviewed for completeness. If you are not a resident of the City of New York or wish to have a plate registered outside of the State of New York listed on your permit, don't forget to mail the appropriate supporting documents. An application is not considered complete until all supporting documents have been received. Complete applications will be forwarded within five (5) working days to HHC's Bellevue Hospital.
